Referral Process and Application Information

Information for Professionals/Referral Sources

To stay at Faith’s Lodge, each couple or family must receive a referral from a member of their healthcare team or faith community, a mental health practitioner, or an approved support organization, such as the Ronald McDonald House.

There are two ways professionals can make a referral for a couple or family:

  1. Call Faith’s Lodge at 715-866-8200 to speak directly to a staff member

  2. Download the appropriate application and fill-in the referral source assessment and contact information. Pass along the application to the interested couple or family, and ask them to complete the remainder and submit to Faith’s Lodge. (If they need assistance, please offer to help with the application process.)

If we have not had phone contact with you prior to receiving the application, a follow-up call will be placed to you once the application is received.

  • Once your application is received and reviewed, you will be contacted by a staff member to confirm the availability of your preferred date of stay or to discuss being placed on a waiting list.  Following this discussion, a welcome packet will be sent that provides specific information about staying at Faith’s Lodge.
